Patricia Ann (Cook) Hayduk of Valparaiso died Monday, Nov. 24, 2003 in
Chicago after a battle with ovarian cancer. She was 49.
Born Feb. 26, 1954 in Michigan City, she was the daughter of Clarence and
Edna (Pritchett) Cook, who preceded her in death.
On Nov. 5, 1977 in Chesterton, she married John Hayduk, who survives in
Valparaiso.
She is also survived by sister Mary Lynn Kinsel (husband Phil) of
Indianapolis; brother-in-law Daniel Miller (wife Kathy) of Marietta,
Georgia; nephew Marc Miller of Atlanta; niece Carolyn Miller of Crested
Butte, Colo.; aunt Ruth Trudea of Olympia Fields, Ill.; cousins Sharon Blake
(Jack) of Rolling Prairie, Thomas Bleck of Charlottesville, Virg., and Jack
Christensen (Claudia) of Michigan City; mother-in-law Dorothy Hayduk of
Valparaiso; brother-in-law Michael Hayduk (Patricia) of Munster;
sister-in-law Elizabeth Hayduk of Chicago; and nieces Aubrey and Matey
Velasquez of Munster.
In addition to her parents, she was preceded in death by sister Candace L.
Miller of Marietta.
A 1976 graduate of Indiana University/Purdue University-Fort Wayne with a
degree in dental hygiene, she practiced clinically for 27 years in Porter
and Lake counties. She was a member of St. Paul Roman Catholic Church in
Valparaiso, the American Dental Hygienists Association, the Indiana Dental
Hygienists Association and the Northwest Indiana Dental Hygienists
Association, where she served as treasurer and where she was named Dental
Hygienist of the Year for 1996-97. She was a lifetime member of the Indiana
University Alumni Association, the Indiana University/Purdue University-Fort
Wayne Alumni Association, the Indiana University School of Dentistry Alumni
Association, and the National Park Conservation Association.
Wake services will be on Sunday, Nov. 30 from 3 to 9 p.m. at White-Love
Funeral Home, 525 S. 2nd St., Chesterton. A rosary will be recited at 8 p.m.
A Mass of Christian Burial will be at 11 a.m. Monday, Dec. 1 at St. Patrick
Catholic Church, 638 N. Calumet Road, Chesterton with Rev. James Meade and
Rev. Lawrence Heeg concelebrating. Interment will immediately follow in
Chesterton Cemetery.
Memorials may be made to the Indiana University/Purdue University-Fort Wayne
Dental Hygiene Program.
