Proposals
Notice is hereby
given the Porter County Municipal Airport Authority (PCMAA) will receive
sealed proposals for the East End Hangar Sanitary Sewer and Water Service
project until 8:00 AM, local time, on Tuesday, May 17, 2011, at the PCMAA
Terminal Building, located at 4101 Murvihill Road, Valparaiso, Indiana.
A mandatory
pre-qualification conference will be held at 2:00 PM, local time, on
Tuesday, May 10, 2011, at the PCMAA Terminal Building.
General Project
Description
This project
consists of the following three separate work items:
1) Furnish and
install approximately 190 l.f of 12” DIP water main and associated
appurtenances along Murvihill Road.
2) Furnish and
install approximately 2,200 l.f. of 1” type “K” copper service line and 340
l.f. of 6” C-900 PVC sanitary sewer line and associated appurtenances behind
hangars between taxiways “R” and “S”.
3) Furnish and
install approximately 480 l.f. of 6” DIP water main, one fire hydrant and
330 l.f. of 6” C-900 PVC sanitary sewer line and associated appurtenances
behind hangars along the east side of taxiway “S”.
General Project
Requirements
1) Award of
Contract. Proposals will be publicly opened and read aloud at the Regular
PCMAA Board Meeting held at the PCMAA Terminal Building on Tuesday, May 17,
2011. Proposals shall remain in open and in full force for a period of 60
days after opening. The PCMAA anticipates completing all three work items of
this project in 2011. The PCMAA reserves the right to waive any
informalities, reject any and/or all proposals for any reason or no reason.
The PCMAA will award a contract, or contracts, for the work, based on
proposals received, to the lowest responsible and responsive contractor,
that will, in the sole judgment of the PCMAA, best serve the interests of
the PCMAA.
2) Project
Schedule. Project shall be substantially complete within 60 days of
receiving Notice-to-Proceed, and shall be fully completed within 90 days of
receiving Notice-to-Proceed.
3) Pay Requests
and Retainage. Pay requests shall be processed and paid monthly based on
work completed to date. Retainage shall withheld from monthly pay requests
and released within 30 days of acceptance of final project completion.
Retainage shall be 10% of the amount of the monthly pay request.
4) Contractor’s
staging area. The contractor may use the area inside the east end gate for
equipment and material storage and other project staging needs.
5) Proposal
Requirements
Each proposal
shall be enclosed in a sealed envelope and addressed as follows:

Each proposal
shall include the following information
1) Contractor’s
name, business address, telephone and fax numbers and e-mail address.
2) Principal
officer or owner and proposed project superintendent name(s).
3) Identification
of proposed subcontractors, if any.
4) References for
similar type work.
Payment and
performance bond in the amount of 100% of the proposed contract amount. Bond
shall be in full force and effect for a period of 12 months after acceptance
of the work by the PCMAA.
Proof of
liability insurance.
Fully executed
non-collusion affidavit, Form 96.
The attached bid
forms, filled out in their entirety.
Each proposal
shall include all materials and labor necessary to complete the work as
shown on the project plans.
6) Project plans.
Project plans are available from NPES, LLC, located at 158 South Napoleon
Street, Suite 105, Valparaiso, Indiana, 46383, (219)462-8050, for a
non-refundable fee of $30.
Technical
Specifications
I ) Sanitary
Sewer
Sanitary sewer
construction and materials shall meet the specifications of the City of
Valparaiso. These specifications are available on-line and at Valparaiso
City Hall. (www.valparaisoutilities.org) Questions regarding these

Sanitary sewer
pipe shall be 6” diameter, C-900, PVC pipe Individual service taps shall be
made with a 6” x 6” x 6” “wye” fitting.
Individual
service stubs shall be plugged and marked per the details shown on the
plans.
Number 6, bare,
copper tracer wire shall be installed with the sanitary sewer pipe.
II) Water Main
Water main and
service line construction and materials shall meet the requirements of the
City of Valparaiso. These specifications are available on-line and at

1” diameter water
service lines shall be continuous, 1” diameter, type-K copper pipe.
12” diameter
water main shall be pressure class 350 ductile iron pipe.
6” diameter water
main shall be thickness class 52 ductile iron pipe.
12” and 6” gate
valves shall be Clow.
Fire hydrant
shall be U.S. Pipe Metropolitan M-94 w/mj shoe.
1” corp. stops
shall be Ford FB 1000-4Q Compression.
1” shut-off
valves and boxes shall be Ford B44-444-Q Compression and Tyler.
III)
Miscellaneous
All disturbed
areas shall be finish graded to match surrounding areas.
All disturbed
areas shall be seeded upon completion of finish grading.
Existing wells
shall be abandoned.
