The Duneland School Corporation (DSC) has set tentative dates for H1N1 vaccinations at Chesterton Middle School and Chesterton High School.

These dates could change based on the availability of the vaccine from the Porter County Health Department, DSC said.

The tentative date:

•Chesterton Middle School, Monday, Nov. 9.

•Chesterton High School, Wednesday, Nov. 11.

Parents will be contacted the night before the scheduled immunization if the clinic will be delayed due to a shortage of vaccine, DSC said.

“It is important that the consent forms be filled out and returned by the deadline date so that the Porter County Health Department can determine if they’ll have enough vaccine available,” DSC said. The vaccination information and consent form can be found on the Duneland Schools website at www.duneland.k12.in.us/heathservices/healthhome.html

Consent forms for CHS will be distributed to CHS students today and must be returned by Monday, Nov. 9.

Meanwhile, a makeup day for 3 p.m. Wednesday, Nov. 11, at the CHS cafeteria has been scheduled for any student in any Duneland School who missed the vaccination. The student must have a signed consent form in order to be vaccinated.
